Jehovah's Witnesses and Blood Issues
Jehovah's Witnesses are well known for their refusal of blood transfusions. But how much do you really know? What can or can't they have, are epidural blood patches acceptable and what rights do parents have to refuse their children blood transfusions? This article discusses these issues as well as giving background on who are Jehovah's Witnesses and what they believe in. It also covers the area of consent in both children and adults according to New Zealand law.

The γ- cyclodextrin derivative sugammadex (trade name Bridion) has been recently introduced as a novel agent to reverse the action of rocuronium. [4] Sugammadex has been in use since 2009 in many European countries; however, it was turned down for approval twice by the US FDA due to concerns over allergic reactions and bleeding, [5] but finally approved the medication for use during surgical procedures in the United States on December 15, 2015. [6] Neostigmine can also be used as a reversal agent of rocuronium but is not as effective as sugammadex. Neostigmine is often still used due to its low cost compared with sugammadex. [7]

Medication suffixes such LA, CR, CD, ER, XL and SR have no standard interpretation that would give a clear meaning to indicate release properties or dosing frequency. This is an area of controversy and can lead to medication errors.

Pharmacology can get confusing to just rely on end spellings of medications for all of the different chemical, generic (nonproprietary), and trade (proprietary) names. Some may also hear similar and not realize they are very different: Inder al (propran ol ), Albuter ol , Topr ol and Lopress or (metopr olol ), Sot alol or Sotal ex and Sota cor , Normo dyne (labet alol ), Coreg (carved ilol )

Then you have a list of ACE Inhibitors which some say the il is common: lisinopril, enalapril, ramparil, Zestril plus Altace but one might also see Proventil
Ny statin is an antifungal drug
It is best to LEARN pharmacology and not look for shortcuts or rely on memorization.

On the other hand, we feel that endovascular therapies may be potentially more beneficial as a treatment option. Encouraging results have been reported with intra-arterial administration of papaverine and angioplasty of accessible spastic vessels. Timing of endovascular treatment is critically important to be effective. Intervention should be performed soon after it is apparent that a patient is progressing or failing to improve despite maximal medical therapy and before the onset of cerebral infarction. Indeed, cerebral angiography with the possibility of angiopalsty has become a routine part of our protocol in the management of symptomatic vasospasm. Figure 1 shows an example of a patient with symptomatic basilar artery vasospasm who made a significant recovery (from obtundation to following commands) after angioplasty.
